An application can be mailed directly to you or we can assist you with filling out forms at our front desk.

1. Birth Certificate

2. Social Security Card

3. Immunization Record (updated)

4. Special Education Letters or ARD papers from previous school

5. Parents Drivers License (High School students only)

6. Utility Bill or Phone Bill (most recent month)

7. all TAAS / TAKS Scores

8. Name of the school district you live in

9. Name of the Public School you child would attend
